REFERRALS

How to Get Referred to a Specialist

Specialist visits in Ontario generally require a referral from a primary care clinician. ProfileMD is a directory and education resource; we do not process, send, manage, or guarantee referrals.

1. Start with primary care

Contact your family doctor, nurse practitioner, a walk-in clinic, or your current care team to discuss your symptoms and determine if a specialist referral is appropriate.

2. Prepare before requesting

Clearly summarize your main reason for the referral, your symptom timeline, relevant past test results, your current medication and allergy lists, and any prior specialist notes.

3. After the referral is sent

Ask your primary care provider where the referral was sent, what the expected wait time or booking process is, and which clinic to contact if you haven't heard back.

4. If symptoms are urgent or worsening

Contact your current clinician or use urgent/emergency services. For medical emergencies, call 911 or go to the nearest emergency department immediately.

How to Prepare for a Visit

Follow these standard steps to prepare for your clinic appointments. Use clinic-approved secure tools when provided by your doctor's office.

Before the visit

  • List your top 3 concerns and symptom timeline.
  • Prepare a complete medication and allergy list.
  • Log recent home readings (blood pressure, glucose, weight, peak flow, oxygen saturation, temperature).
  • Gather prior reports, imaging, lab results, or hospital discharge summaries.
  • Complete any forms requested directly by the clinic.

During the visit

  • Ask clearly what the treatment or follow-up plan is.
  • Ask which symptoms should prompt urgent care.
  • Note any medication changes.
  • Confirm timing for follow-up appointments.
  • Ask who to contact for test or referral results.

After the visit

  • Keep discharge or post-visit instructions accessible.
  • Complete required tests or forms through clinic-approved tools.
  • Update your personal medication list immediately.
  • Book follow-up visits as directed.
  • Do not send PHI through public ProfileMD channels.

Emergency vs. Non-Emergency Guidance

ProfileMD does not triage symptoms and cannot tell you which level of care you need. When in doubt, seek professional medical evaluation.

Medical Emergency

  • Chest pain or tightness
  • Stroke symptoms
  • Severe shortness of breath
  • Major trauma or injury
  • Severe allergic reaction
  • Loss of consciousness or sudden confusion
